Receive updates about the PCPR program
Participation in the PCPR program involves evaluating your primary care practice across a range of accountabilities including whole-person care, health literacy, and behavioral health integration. Primary care practices will be assigned a level from 1 to 3 based on the results of the PCPR evaluation. The practices that participate in the PCPR program will be recognized for their commitment to providing whole-person, integrated care that results in improved patient and population outcomes.
The goal of the PCPR program is to move primary care practices towards a common definition of advanced primary care and serve as a framework for practice transformation. Data from participating practices will help us understand the capabilities and challenges facing primary care in Washington State and identify potential interventions and support.

Office hours are for practices who have questions for HCA about the PCPR program or who need help completing an application. Practices are encouraged to reach out for individual support if they are unable to attend the office hours.
