Apple Health Core Connections
Youth in foster care are enrolled in a single statewide managed care program to ensure continuity of medical care while they are under dependency. There are some exceptions, including American Indian/Alaska Native youth and youth who have other qualifying coverage.
The Apple Health (Medicaid) Foster Care managed care plan is called Apple Health Core Connections and is administered by Coordinated Care (CC). This plan provides coordinated health care services for:
- Children and youth in a foster care dependency.
- Extended foster care.
- Adoption support.
- Unaccompanied Refugee Minors (URM).
- Adult alumni of the foster care program.
