In Washington State, D-SNP is now known as Apple Health Medicare Connect. This name change better communicates that Apple Health Medicare Connect is only available to clients who are enrolled in both Apple Health (Medicaid) and Medicare coverage (referred to as dual-eligible).

Medicare Advantage (MA) D-SNP care coordinator training
This video training provides a high-level overview of Washington state's long-term care (LTC) system and reinforces the collaborative responsibilities of MA D-SNP care coordinators, MA plan partners, Home and Community Services (HCS), and Area Agencies on Aging (AAAs) in delivering integrated, person-centered care for individuals who are dually eligible for Medicare and Medicaid.
