To reduce or decline your employee-paid LTD insurance:
- Submit the Long-Term Disability (LTD) Enrollment/Change Form to your payroll or benefits office.
When does the change take effect?
A request to reduce or decline the employee-paid LTD insurance will take effect the first day of the month following the date the employer receives the form.
Note: If you later decide to enroll in or increase coverage, you will have to provide evidence of insurability. An increase in coverage takes effect the day the evidence of insurability is approved.
Once you enroll in a FSA or DCAP for a plan year you cannot change your election unless you experience a special open enrollment or end employment.
