Primary Care Practice Recognition

PCPR participation

Participation in the PCPR program involves evaluating your primary care practice across a range of accountabilities including whole-person care, health literacy, and behavioral health integration. Primary care practices will be assigned a level from 1 to 3 based on the results of the PCPR evaluation. The practices that participate in the PCPR program will be recognized for their commitment to providing whole-person, integrated care that results in improved patient and population outcomes.

PCPR vision

The goal of the PCPR program is to move primary care practices towards a common definition of advanced primary care and serve as a framework for practice transformation. Data from participating practices will help us understand the capabilities and challenges facing primary care in Washington State and identify potential interventions and support.

Intended outcomes

• Establish eligibility for participation in alternative payment models and move away from the fee-for-service model of care.
• Inform practices about support like transformation funding, learning resources, and technical assistance including webinars, coaching, and more.
• Improve the quality of care, health outcomes, and health equity for Washingtonians.

Apply for recognition

Steps to apply:

1. Review the Practice Accountabilities and Evaluation Guide. The guide details the primary practice accountability topics and the required elements for each level.
2. Complete the accountabilities workbook. Completing the workbook will calculate the PCPR level for your practice.
   • If your practice has active Patient-Centered Medical Home (PCMH) recognition, email the PCPR program for the alternate accountabilities workbook.
3. Set up a PCPR identifier. Email HCA PCPR to receive a PCPR identifier. Write “PCPR Identifier Request” in the subject line. You will receive a PCPR identifier. In the email, include:
   • Your practice name.
   • Practice address.
   • Contact person, including phone number and email address.
4. Set up your SecureAccess Washington (SAW) account. While you wait for your PCPR identifier, sign up for a SAW account.
   • View instructions for setting up a SAW account if you do not have one.
5. Apply for PCPR recognition. Log into HCA Support to submit your application. The recognition evaluation and level are for each practice site. If an organization operates multiple practice sites, a separate application must be submitted for each site.
