Cancel your coverage
Find out how to defer or cancel (terminate) your PEBB retiree insurance coverage or term life insurance.
You may choose to defer (postpone) your enrollment in a PEBB retiree health plan at the time you become eligible for PEBB retiree insurance or after you enroll. See How do I defer my coverage for more information.
Note: If you defer your PEBB retiree health plan coverage, you must maintain continuous enrollment in other qualified medical coverage to reenroll in a PEBB retiree health plan later on. For additional information, see deferring coverage.
To cancel (terminate) all or part of your PEBB retiree health plan coverage submit your request in writing in one of the following ways:

When will coverage end?
Your requested coverage will end on the last day of the month in which the PEBB Program receives your written notice. If your written notice is received on the first day of the month, coverage will end on the last day of the previous month, unless you are enrolled in a Medicare Advantage plan. See exceptions below:
- If you are requesting to cancel dental coverage only, you must have been enrolled for at least two years and the cancellation will include any covered dependents.
- If you are cancelling (terminating) medical coverage and you or a covered dependent are enrolled in a Medicare Advantage plan, you must also send a completed PEBB Medicare Advantage Plan Disenrollment Form (form D) to the PEBB Program. PEBB retiree health plan coverage will end on the last day of the month when the Medicare Advantage Plan Disenrollment Form (form D) is received.
Note: If you cancel (terminate) your PEBB retiree health plan coverage, you cannot enroll again later unless you regain eligibility for PEBB retiree insurance coverage, for example, by returning to employment in a PEBB or SEBB benefits-eligible position. If you cancel (terminate) coverage, any enrolled dependents’ coverage will be terminated as well.
To cancel your retiree term life insurance, you must complete MetLife’s Cancellation of Life Insurance - Retiree form and send it to:
MetLife Recordkeeping Center
PO Box 14406
Lexington, KY 40512-4406
Note: If you cancel your retiree term life insurance, you may not reenroll unless you regain eligibility for life insurance at a later date for example by returning to employment in a PEBB or SEBB benefits-eligible position.
