As an employee or retiree, your dependents may be able to enroll in or defer (postpone) enrollment in PEBB retiree insurance coverage as your survivors. Learn about their options and what they'll need to do to enroll.

To be eligible, your surviving spouse, state-registered domestic partner, or child must:
- Immediately receive a monthly retirement benefit.
- Submit the required forms and first premium payment by the deadlines described under How do survivors enroll or defer enrollment?

What if I am a survivor of emergency service personnel?
Survivors of emergency service personnel killed in the line of duty may be eligible to enroll in or defer (postpone) enrollment in PEBB retiree insurance coverage if they meet the eligibility and procedural requirements outlined in WAC 182-12-250.

Survivors of eligible employees and retirees
To enroll in or defer enrollment in PEBB retiree insurance coverage, the PEBB Program must receive the PEBB Retiree Election Form (form A) and any other required forms within the following timelines. (See the Retiree Enrollment Guide and visit the Completing the retiree forms webpage for a complete list of possible forms.)
For an eligible survivor of a(n):
- Employee: No later than 60 days after the employee’s death, or the date the survivor’s PEBB, educational service district, or SEBB insurance coverage ends, whichever is later.
- Retiree: No later than 60 days after the retiree’s death.
- Elected or full-time appointed official: No later than 60 days after the official’s date of death, or the date the survivor’s PEBB insurance coverage ends, whichever is later.
Exceptions to this 60-day timeline apply if you enroll in a Medicare Advantage or Medicare Advantage Prescription Drug plan. Learn more about Medicare and PEBB benefits.
Survivors of emergency service personnel
To enroll in or defer enrollment in PEBB retiree insurance coverage, the PEBB Program must receive the PEBB Retiree Election Form (form A) and any other required forms no later than 180 days after the later of these events:
- The death of the emergency service worker.
- The date on the letter from the Department of Retirement Systems or the Board for Volunteer Firefighters and Reserve Officers that informs the survivor they are eligible.
- The last day the survivor was covered under any health plan (including COBRA coverage) through the emergency service worker’s employer.

- For surviving spouse or state-registered domestic partner: indefinitely.
- For other dependents: may continue enrollment until they are no longer eligible under PEBB Program rules (for example, the end of the month in which they turn age 26).
The survivor must pay monthly premiums and applicable premium surcharges as they become due.
When a dependent becomes eligible as a survivor, they will be enrolled under their own account. In most cases, there will be no gap in coverage.
