About the Primary Care Transformation Initiative (PCTI)

The PCTI brings together health care providers, health plans, employers, purchasers, and state agencies to improve primary care. PCTI works to expand access to comprehensive and coordinated care and strengthen primary care practices across Washington. Primary care is the foundation of our health care system. Research shows that greater use of primary care is associated with:

  • Better health outcomes
  • Improved patient experience
  • Lower overall health care costs
  • Reduced clinician burnout

PCTI programs and partnerships

Multi-Payer Collaborative (MPC)

The Washington MPC is a group of payers who work together to improve primary care delivery and payment. MPC currently focuses on:

  • Improving health outcomes through primary care.
  • Expanding access to high-quality comprehensive primary care.

Multi-payer Collaborative Primary Care Transformation Model (PCTM)

The PCTM is a set of strategies developed by Washington health care providers, health plans, and purchasers to strengthen primary care throughout the state. The model includes:

  • Aligning payment methodologies
  • Increasing investment in primary care
  • Supporting primary care practices to improve how they deliver care

Primary Care Practice Recognition program

The Health Care Authority’s (HCA) Primary Care Practice Recognition (PCPR) program provides an evaluation framework for participating primary care practices which determines a recognition level from 1 to 3.
