Dental plans and benefits
PEBB members and their dependents have three dental insurance plans to choose from: Uniform Dental Plan, DeltaCare, and Willamette Dental. Learn about each plan and find resources to help you choose. If you cover dependents, everyone must enroll in the same dental plan.

Plan options
Explore each dental plan to see which plan is best for you. If you do not select a plan, you will be automatically enrolled as a single subscriber in Uniform Dental Plan.

Visit the following PEBB Virtual Benefits Fair webpages to find information on benefits coverage (full and summary) for each plan. To see in-depth coverage options, review each plan's benefits booklet (Certificate of Coverage).
- Uniform Dental Plan: This plan is administered by Delta Dental and has a large, nationwide network of providers.
- DeltaCare: This plan is also administered by Delta Dental, but has a limited network of providers and is only in Washington.
- Willamette Dental Plan: This plan has a limited network of providers, but is available in Washington, Oregon, and Idaho.

DeltaCare and Willamette Dental are managed care plans
With a managed care plan, you must choose a primary dental provider within their networks. If you do not choose a primary dental provider, one will be chosen for you. These plans will not pay claims if you see a provider outside of their network.
Is your current provider in the plan's network?
Call your dentist, the dental plan's customer service, or use the dental plan's online directory to verify that your current provider is in network. Make sure you correctly identify your dental plan's network and group number.
Carefully review the selection you make before submitting your enrollment form.
Premiums
If your employer offers this coverage, your dental insurance premiums are paid for by your employer.

Check each dental plan's provider directory to learn about their providers and see if your current provider is in network.
Insurance ID cards
Willamette Dental does not use ID cards. Uniform Dental Plan and DeltaCare do not mail ID cards, but you can download one from their website.
