Who is eligible?

Generally, subscribers and their spouse or state-registered domestic partner enrolled in a PEBB medical plan can participate in SmartHealth.

However, only the following subscribers can qualify for the SmartHealth wellness incentive:

  • Employees.
    Are you an employee waiving PEBB medical coverage? If so, you can still access SmartHealth, but you will not qualify for the SmartHealth wellness incentives.
  • Retirees not enrolled in Medicare Part A and Part B.
    Are you a retiree deferring PEBB retiree insurance coverage? If so, you will not have access to SmartHealth and will not qualify for the SmartHealth wellness incentives.
  • PEBB Continuation Coverage (Unpaid Leave) subscribers.
  • PEBB Continuation Coverage (COBRA) subscribers not enrolled in Medicare Part A and Part B.

What is the incentive?

Eligible PEBB subscribers can qualify for a $125 wellness incentive.

To get your $125 incentive, you must qualify in 2024 and be enrolled in a PEBB medical plan as a subscriber in 2025.

How you receive your $125 incentive depends on the type of medical plan you enroll in for 2025.

  • Consumer-directed health plans (CDHPs): A one-time deposit of $125 into the subscriber's health savings account (HSA) in January 2025.
  • All other plans: A $125 reduction to the subscriber's 2025 PEBB medical plan deductible in January 2025.

How do I qualify?

Complete all three steps within the deadlines described below to qualify for the wellness incentive.

  1. Get started. Sign in to SmartHealth.
  2. Take the assessment. Finish the 15-minute SmartHealth well-being assessment and earn 800 points.
  3. Qualify. Join and track activities to earn at least 2,000 total points by your deadline to qualify for the $125 wellness incentive in 2025.

Do you need an accommodation? If you are unable to complete a wellness incentive requirement, contact WebMD (SmartHealth administrator) at 1-800-947-9541. They will work with you to provide an alternative requirement that will allow you to qualify for the wellness incentive or waive the requirement. When is the deadline?

Your deadline to qualify for the $125 wellness incentive depends on the date your PEBB medical coverage becomes effective. Use the table below to find your deadline. You must take the well-being assessment and earn at least 2,000 total points to qualify.
