Find providers

Check with your dental provider to see if they are in the plan's network and group number. Make sure you correctly identify your dental plan's network and group number. You can call the dentist, the dental plan's customer service, or use the dental plan's online directory.

Uniform Dental Plan: You may see any dental provider, but you'll usually pay less out-of-pocket if you go to a provider in the Delta Dental PPO network.
DeltaCare: You may only see providers in the DeltaCare network. You'll have a primary care provider who oversees your care, and authorizes specialist referrals. If the provider you select is not in DeltaCare's network, you will be responsible for the costs.

Uniform Dental Plan (UDP)

Delta Dental of Washington’s preferred-provider organization (PPO) plan. This plan allows you to choose your dentist from the largest state and nationwide networks. Deductible: $50/person / $150/family (Deductible is waived for preventive care and children under age 15.)
Out-of-pocket maximum: $1,750

Use Delta Dental PPO network dentists for the best rates. The PPO network includes over 104,000 dentists nationwide. You may see any licensed dentist within the U.S. for care.

It's best for those who want the most freedom to choose their dentist.

DeltaCare

Delta Dental of Washington’s managed-care (HMO) dental plan. This plan requires you to receive care and specialist referrals only from a DeltaCare network dentist. Coverage will not apply if you see an out-of-network dentist. You pay fixed copays for dental care, with no deductibles and no annual maximum (for most services). No deductible or annual out-of-pocket maximum.

Requires primary care dentist from DeltaCare network.
Primary care dentist must refer you for any specialty care.
No coverage for out-of-network care.
