Form 1095-C is not required to file your 2021 tax return

By late February 2022, the Health Care Authority (on behalf of your employer) will mail 2021 Forms 1095-C to state agency, higher-education, and commodity commission employees enrolled in Uniform Medical Plan (UMP). Employees determined “full-time” under Affordable Care Act regulations will also receive the form.

If you are enrolled in Kaiser Permanente NW or Kaiser Permanente WA, you will receive Form 1095-B directly from the plan.

Do I need From 1095-C to file my taxes?

No. You do not need to wait for either of these 1095 forms to file your tax return, but you should keep them for your records.
