Form 1095-C is not required to file your 2018 tax return
In late February 2019, the Health Care Authority (on behalf of your employer) will mail 2018 Forms 1095-C to state agency, higher-education, and commodity commission employees enrolled in Uniform Medical Plan (UMP). Employees determined “full-time” under Affordable Care Act regulations will also receive the form.
If you are enrolled in Kaiser Permanente NW or Kaiser Permanente WA, you will receive Form 1095-B directly from the plan.
You do not need to wait for either of these 1095 forms in order to file your tax return, but you should keep them for your records. You can prepare and file your return using other information about your health coverage such as your W-2, payroll statements, or insurance cards.
