ACH submitted documents
Throughout the Medicaid Transformation Project (MTP), Accountable Communities of Health (ACHs) will submit several documents that provide information on the planning, implementation, and progress of their selected projects. Certification documents, project plans, and semi-annual reports are required by our special terms and conditions. ACHs are eligible to earn incentive payments upon submission.
