Outcomes improve for clients when systems coordinate
The King County ACH is working to achieve transformative change in the region to prevent disease, improve quality of life and promote well-being. Operating under the county’s Health and Human Services Transformation plan, the ACH incubates and accelerates initiatives to strengthen communities and improve overall population health. Familiar Faces is one of those initiatives.
The Familiar Faces initiative assists people who have been in jail four or more times in a 12-month period and who have a mental health or substance use condition.
Healthier Washington Linkages
Healthier Washington is supportive of the cross-system collaboration demonstrated in the Familiar Faces initiative. Healthier Washington seeks to ensure that people receive coordinated care for physical and mental health and substance use, in a way that works for that person wherever they are.
