ACH submitted documents
Over the course of Medicaid Transformation, ACHs will submit several documents that provide information on the planning, implementation, and progress of their selected transformation projects. These certification documents, project plans, and semi-annual reports are required by our special terms and conditions. ACHs are eligible to earn incentive payments upon submission.
