Long-term disability insurance (continuation coverage only)

You may self-pay basic and optional long-term disability coverage when you are on approved educational leave or called to active duty in the uniformed services.

Eligibility requirements

You may self-pay basic and optional long-term disability (LTD) coverage when you are on approved educational leave or called to active duty in the uniformed services as defined under the Uniformed Services Employment and Reemployment Rights Act (USERRA).

Your personnel, payroll, or benefits office has a definition of educational leave.

Continuing LTD benefits while on continuation coverage

To continue your LTD insurance coverage on while on USERRA or educational leave, fill out the LTD section of the continuation coverage election/change (for Leave Without Pay) form.

If you continue LTD coverage while on USERRA or educational leave

You must self-pay the $2.10 monthly premium (guaranteed through December 31, 2017) for basic coverage plus continue to pay any premiums for optional LTD coverage you choose to keep.

If you choose not to continue optional LTD coverage while on USERRA or educational leave

You must provide evidence of insurability when you regain eligibility and reapply for LTD coverage.

Reinstating LTD insurance when you return to work

Reinstating your LTD coverage when you return to work will differ based on whether or not you continued LTD coverage during continuation coverage.

You do not have to reapply for basic LTD coverage when you return to work. Basic LTD coverage is reinstated automatically the first day of the month you regain eligibility for the employer contribution.

If you were not eligible to continue LTD coverage during continuation coverage
Your basic LTD insurance is reinstated the first day of the month you regain eligibility for the employer contribution. Your optional LTD insurance is reinstated the first day of the month following the date you regain eligibility, to the same level of coverage you were enrolled in before continuation coverage. You do not have to complete and submit any forms.

If you discontinued optional LTD coverage during continuation coverage
You must reapply apply for optional LTD insurance. Complete and submit the Long-Term Disability Enrollment/Change Form to your employer and the Long Term Disability (LTD) Evidence of Insurability Form to Standard Insurance Company no later than 31 days after you regain eligibility for the employer contribution. Your insurance does not become effective until approved by Standard Insurance Company.

If you continued optional LTD coverage during continuation coverage and you return to work immediately following your leave period
If you become eligible for the employer contribution immediately following your leave during the first 29 months, your LTD insurance does not end. You do not have to complete and submit any forms to continue the amount of coverage you had during continuation coverage.

If you continued optional LTD coverage during continuation coverage but do not return to work immediately following your leave period
If you do not immediately return to work after your approved leave period and your insurance ends, you are eligible to reinstate your basic and optional LTD insurance the first day of the month you regain eligibility for the employer contribution as a new subscriber. Complete and submit the Long-Term Disability Enrollment/Change Form to your employer no later than 31 days of becoming eligible for benefits.

If you apply for optional LTD coverage after 31 days you must provide evidence of insurability (proof of good health) and Standard Insurance Company must approve your request before your insurance becomes effective.

If applying after 31 days:

  1. Complete and submit the Long-Term Disability Enrollment/Change Form to your employer’s personnel, payroll, or benefits office.
  2. Complete the Long Term Disability (LTD) Evidence of Insurability Form and submit it to Standard Insurance Company (at the address on the form).

How to change your optional LTD waiting period when you return to work

To increase your waiting period for optional LTD coverage

Complete and submit the Long-Term Disability Enrollment/Change Form to your employer’s personnel, payroll, or benefits office no later than 31 days after you regain eligibility for the employer contribution.

To decrease your waiting period for optional LTD coverage

  1. Complete and submit the Long-Term Disability Enrollment/Change Form to your employer’s personnel, payroll, or benefits office.
  2. Complete the Long Term Disability (LTD) Evidence of Insurability Form and submit it to Standard Insurance Company (at the address on the form).

The decreased waiting period would not become effective until approved by Standard Insurance Company. Otherwise, you will continue to be enrolled in the same level of basic and optional LTD insurance you had during continuation coverage.
