HCA is a member of the Washington Patient Safety Coalition, a group working to reduce medical errors and improve safety in all medical facilities in Washington State. Please help us by completing a survey about your use of medication lists to keep track of all of the prescription drugs, over-the-counter medications, vitamins, and herbal remedies you use.
The survey takes just three minutes to complete, and the information you provide will help us learn how many people use medication lists and how they use them. Your responses will be anonymous unless you choose to identify yourself in the final question.
Medication errors are a major cause of illness and injury. The Washington Patient Safety Coalition will use the survey results to develop strategies to reduce medication errors by patients and health care providers.
